On Monday, March 25, 2024, Alluvion Health announced the renewal of its Project in Assistance in Transition from Homelessness (PATH) program with a new team of professionals. This program is designed to support individuals experiencing homelessness, particularly those struggling with mental illnesses or substance use disorders. The PATH program offers various services such as outreach, engagement, case management, and service coordination to identify eligible individuals, assess their needs and connect them with resources to address their homelessness.
